Patient Services
Information


Our physicians bill separately from the hospital for services that they provide. Diagnostic tests, lasers, and surgeries may also generate a bill from the Oregon Health & Science University. The following policies refer to Casey faculty billing only.


Medicare
We are a participating group with Medicare, which means that we will bill for covered services and receive payment directly from Medicare. You will be responsible for your annual deductible and 20% coinsurance. Medicare does not cover routine eye exams, refractions or cosmetic services, so we may ask for payment for those transactions at the time of your visit. If you have supplemental insurance (Medigap), we will bill them as a courtesy if you provide the billing information to us.


HMO/PPO Plans
If you are insured by a managed care organization, we ask for your participation in acquiring appropriate authorizations for services here. As specialists, most of our services require these authorizations in order for your visit to be covered by your plan. Should you wish to be seen without an authorized referral, we may ask you to pay for your exam at the time of service and ultimately you will be responsible for payment of the bill. Please provide your co-payment at the time of your visit; we accept cash (exact amount please), checks and Visa or MasterCard.


Medicaid
Please show your eligibility card at each visit. Oregon Health Plan (OHP) requires authorization for every specialist visit. We request your assistance in obtaining these referrals from your primary care physician (PCP), as we cannot see you, and may have to reschedule your appointment, if you do not have a valid referral.


Private Insurance
If you are covered by another type of insurance plan, we will bill them as a courtesy for you. Please provide complete information so that we may do so. Many plans make payment based on usual and customary charges. This is an average of your insurance company's typical claims data for all specialties; our charges may not be paid in full. If you are here because of a work injury, please notify the receptionist so that we may file a claim with your employer's worker's compensation carrier.


Uninsured
Please do not let medical care lapse due to the fact that you might not have insurance coverage. We may ask for payment at the time of service and we also accept Visa and MasterCard. Please speak to the receptionist or our business office regarding payment policies for surgery.


Vision Insurance
Ophthalmic services are provided for medical conditions as well as routine vision care. Please understand that health insurance does not cover routine vision services. If you have a vision insurance plan, please be sure to provide that billing information.


Insurance policies and health plans are very complex and individual. We make every effort to process your medical claims as quickly as possible. However, it is impossible for us to know the details of every health plan. Your plan administrator is the best source for information regarding your individual insurance coverage. Thank you for your understanding.
